| บทคัดย่อ: โครงการพิเศษนี้เป็นการสำรวจถึงแญหาจากากรใช้ยาวาร์ฟารินของผู้ป่วยนอกโรงพยาบาลบำรุงราษณฎร็ โดยทำการเก็บข้อมูลย้อนหลังจากเวชระเบียนและการสัมภาษณ์ผู้ป่วยซึ่งสึบค้นผู้ป่วยที่ได้รับยาวาร์ฟารินจากใบสั่งยาในระหว่างเดือนพฤษภาคม ถึง สิงหาคม 2543 จากผู้ป่วยที่รับยาวาร์ฟาริน 146 คน เก็บข้อมูลจากเวชระเบียนได้ 50 คนและมีโอกาสสัมภาษณ์ 24 คนโดยเป็นผู้ป่วยชาย 27 คน(ร้อยละ 54.00)และผู้ป่วยหญิง 23คน(ร้อยละ46.00)พบปัญหาที่เกิดขึ้นจาการใช้ยาวาร์ฟาริน 55ปัญหา ในผู้ป่วย28 คน ปัญหาที่พบส่วนใหญ่คือ ค่า INR ผิดปกติจำนวน 29 ปัญหา จากผู้ป่วย17 คน สาเหตูของปัญหาที่เกิดขึ้นส่วนใหญ่มาจากการปรับขนาดยาจำนวน 18 คน การไม่ให้ความร่วมมือในการใช้ยา 7 คน สำหรับปัญหาทีคาดว่าอาจเกิดขึ้นจาการใช้ยาวาร์ฟาริน พบ 254 ปัญหา ในผู้ป่วย 48 คน ปัญหาส่วนใหญ่เกิดจากปฏิกิริยาระหว่ายาที่คาดว่าน่าจะเกิดขึ้นจำนวน 47 คน จะเห็นได้ว่าแญหาเนื่องจากการใช้ยาวาร์ฟารินอาจสามารถป้องกันได้ ดังนั้นการให้ความรู้ คำแนะนำแก่ผู้ป่วยและการคัดกรองปฏิกิริยาระหว่างยา-ยา หรือ ยา-อาหาร ตลอดจนการติดตามผลการรักษา ควรดำเนินการเพื่อให้ผู้ป่วยได้รับผลประโยชน์สูงสุดจากการใช้ยา |
| abstract: This project is a survey study of problems during warfarin therapy of outpatients at bumrungrad hospital.Data was collected retrospectively from medical reconds and patient interviiew.The patients who used warfarin were selected from hospitol database between May and August 2000. there were 146 patients who used warfarin. Fifty patients(27 males and 23 females)were reviewed and only 24 patients could be interviewed. Fifty-five warfarin related problems were found in 28 patients the most cimmon problems were abnormal INR:29 problems in 17 patients.Most of them resulted during warfarin-related problems were found 254 episodes in 48 patients,which those problems were caused by potential drug-drug interaction in 47 patients. The problems during warfarin therapy could be prevented,so that education,counselling, screening drug-drug or drug-food interaction and monitoring outcomes of warfarin therapy could be performed in order that the patients will get the highest benefit from drug usage. |
